Department of Pediatric Surgery
Department of Pediatric Surgery provides service at Hacettepe İhsan Doğramacı Children's Hospital. At our hospital, diagnosis and treatment of congenital or acquired gastro-intestinal tract, liver and biliary tract, urinary tract, lung and chest diseases covered by modern pediatric surgery as well as pediatric liver transplantation and endoscopic surgery are performed by expert staff personnel, particularly by the academic staff of Department of Pediatric Surgery of Hacettepe University Faculty of Medicine.
Department of Pediatric Surgery has 4 laparotomy sets, 4 biopsy sets, 3 special urological surgery sets, 1 sternotomy set, 1 organ transplantation set, 1 small and 1 large special sets as well as 2 esophagoscopes, 1 colonoscope, 1 pediatric laparoscopy and thoracoscopy set, 1 pediatric cystoscopy set, 1 pediatric bronchoscopy set and compatible devices with these gadgets including video imaging system, esophageal manometry, anorectal manometry and video-urodynamicscs equipment and services are provided in one outpatient clinic, two operating rooms and two inpatient units, one of which consists of 27 beds for older children while the other consists of 20 beds for neonates.
Department of Pediatric Surgery was established in 1963 by Dr. Akgün Hiçsönmez and 49 pediatric surgery specialists had trained until 2003. The majority of these specialists have assumed academic roles at other universities, established pediatric surgery departments and trained a great number of pediatric surgeons.
Chest, stomach-intestinal canal, reproductive and urinary tract oriented endoscopic and open surgeries as well as endoscopic diagnosis and treatment procedures are performed in Pediatric Surgery Subdivision.
Surgeries
In Department of Pediatric Surgery, surgeries are performed under local or general anesthesia.
Most common surgeries performed under local anesthesia are suturing, dispose of abscess contents, skin and subcutaneous tissue biopsy, muscle and nerve biopsies, nevus removal and lymph node biopsy. Such surgical operations are performed in the Small Surgical Intervention Room, located in Pediatric Surgery Outpatient Clinic Hall.
Surgical operations performed undergeneral anesthesia can be analyzed in two groups; outpatient surgeries and inpatient surgeries.
Outpatient Surgeries
Before such surgeries, the patient is sent to operating room after being controlled in the outpatient clinic, and then after operation is allowed to go home on the same day. Major outpatient surgeries are as follows:

Inguinal area surgeries: Hernia, hydrocele, undescended testicle, varicocele, cord cyst,

Head and neck region surgeries: Lymph node biopsy, bronchial residual surgeries,

Central catheter placement, port catheter insertion,

Perineal area operations: Drainage of perianal abscess, perianal fistula, hemorrhoidectomy, anal dilatation, urethral dilatation.
Considerations before outpatient surgeries

Please accompany your child while she/he gets undressed and puts on special clothing.

Please make sure that there are no accessories such as watches, necklaces, rings, earrings, etc. on your child.

Please return to the waiting lounge, while the child is taken inside with a stretcher.

For the surgery information please keep up with the screen which is situated in the waiting lounge.

After being kept in the reanimation unit for a while after the surgery, the child will be taken to a different hall where his/her relative can be with him/her.

When the child is entirely awake, you will be allowed to leave the hospital.

Necessary information will be given about your child’s diet and control procedure after the surgery.

Inpatient Surgeries
Apart from the above mentioned surgeries, treatments of congenital or acquired breast, gastro-intestinal tract, liver and biliary tract, reproductive and urinary tract problems, as well as organ transplantations, laparoscopic and thoracoscopic practices and certain endoscopic examinations are performed by hospitalizing patients.

Esophagoscopy is the process of visual examination of esophagus through a divice called esophagoscope and it is performed on children under general anesthesia. Esophagoscope is made of metal tube with an inner lens system that makes the front parts visible by refracting beams coming from the light source. Esophagoscopy is used to evaluate the inner section of esophagus, to collect tissue samples when needed, to remove foreign bodies from esophagus and to assess congenital esophagus diseases.
Esophagogastroscopy is the procedure of examining esophagus and stomach through a device called esophagogastroscope and it is performed on children undergeneral anesthesia. Esophagogastroscope is an advanced device which may have two or three channels depending on its type, and has a tail end that can be bent towards various directions and particular angles and is lightened via a light source. Esophagogastroscopy is performed in order to examine esophagus, stomach and, when needed, beginning parts of duodenum and small intestine to collect samples, to remove foreign bodies, and to administer medication enlarged blood vessels in the esophagus and stomach.

Esophageal dilatation is the process of expansion of esophageal stricture by using various bougies. It is performed on children under general anesthesia. The expansion process is performed by moving the plugs from the mouth to the stomach, or vice versa.

Bronchoscopy is a procedure of visual examination of all respiration tracts starting from throat with a device called bronchoscope. It is performed on children under general anesthesia. Bronchoscope is a device, made of an outer metal tube, an inner lens system that makes front parts visible by refracting the beams of light source with various angles. It is used in order to evaluate respiratory tract, to collect sample, to remove foreign bodies and to examine the connections between respiratory tract and esophagus. Usually, the procedure is prepared by hospitalizing the patient and preparing in the service. Outpatients should comply with the following recommendations.

Rectosigmoidoscopy is the procedure of visual examination of the last 15-20 cm of large intestine with a device called rectosigmoidoscope. It is performed on children undergeneral anesthesia. Rectosigmoidoscope is made of a metal tube with an inner lens system that lightens inside. The procedure is performed in order to examine the last part of large intestine, to investigate the cause of bleeding, to collect sample and to remove polyp.

Colonoscopy is a procedure of visual examination of all parts of large intestine and sometimes the last section of small intestine with a device called colonoscope. It is performed on children under general anesthesia. Colonoscope is a tube-shaped device with a flexible tail end that can be bent towards different directions and particular angles. It is performed to examine the entire large intestine and sometimes the last section of small intestine, to collect sample and to remove polyp.

Cystoscopy is a procedure of visual examination of lower urinary tract with a device called cystoscope. It is performed undergeneral anesthesia. Cystoscope is made of an outer metal tube in various thicknesses depending on the age and an inner lens system. Cystoscopy is performed to evaluate the lower urinary tract, to collect sample, to remove stone, to take X-ray in order to monitor upper urinary tract, to treat urinary tract obstructions, to administer medication for the treatment of urine leakage from urinary bladder to kidneys, to investigate connections between the urinary tract and large intestine or to evaluate the connections of female urinary tracts and reproductive tracts and to check the congenital or acquired bladder or tract expansions in urinary tract.

Laparoscopy is a procedure of visual examination of intra-abdominal organs with a device called laparoscope which is placed into abdominal cavity. It is performed undergeneral anesthesia. Laparoscope is made of an imaging system that is connected to a lens into the metal tube which is placed into the abdominal cavity. Laparoscopy can be used to diagnose and treat children. Intra-abdominal organ and lymph node biopsies, gall bladder surgeries and certain surgeries of stomach, intestine, spleen and undescended testis can be performed on children by using laparoscopy.

Thoracoscopy is a procedure of visual examination of thoracic organs with a devicecalled thoracoscope that is placed into the thoracic cavity. It is performed undergeneral anesthesia. Thoracoscope is made of a metal tube placed into the thoracic cavity, a lens placed into the tube and an imaging system that reflects the captured image to the monitor. It can be used to clean pleural effusionsand to relieve the lungs, to remove tumor focuses from lungs or to collect samples from them and from lymph glands developing in the thoracic cavity or lungs and it can also be performed in certain lung and esophagus surgeries.

Esophageal Manometry is used to examine the characteristics of pressure values obtained from different areas of esophagus and pressure waves that occur in the esophagus while swallowing. It is performed on children after administering a minimal sedation. After the patient is given a specific pressure catheter for measurement, pressure values are recorded on computer with relevant software and results are edited on computer before evaluation. Esophageal manometry is used for congenital and acquired diseases such as achalasia related with swallowing disorders, other motor function disorders of esophagus, central nervous system diseases, certain metabolic diseases and connective tissue diseases.

Anorectal manometry is an examination method that is used to obtain information about the functional status of the last section of large intestine by analyzing pressure values obtained from different areas of the last section of the large intestine. It is performed in children by applying minimal sedation, if required. Pressure values obtained by placing bubbled pressure measuring catheter from the anus, are transferred to the computer and the results are evaluated. It is used in the diagnosis and monitoring of several congenital and acquired large intestine diseases in which the indications of constipation and fecal incontinence are seen.

Urodynamics is a method used to examine the function of lower urinary tract. It is performed on children after applying minimal sedation, if required. In this method, pressure measuring catheters are placed in urinary tract and anus, and surface electrodes are affixed to the skin. Pressure changes and muscle activities occurre while catheter is giving liquid to the urinary bladder at constant speed and during urination after storage are recorded to the computer. The rates of urine flow and muscle activity can be also measured by urinating the patient to a particular pot. Urodynamics is used for diagnosis and follow up of children born with closed anus, congenital or acquired spinal cord diseases, recurrent urinary tract infections, patients having complaints such as urine leakage from urinary bladders to the kidneys and urinary incontinence or inability to urine, children whose urinary bladders are open congenitally but closed surgically, children who are suspected to have adjustment disorders in lower urinary tracts, and congenital or acquired strictures of urinary tract.
Urodynamics appointment
After the patient is examined in Pediatric Surgery Outpatient clinic, the appointment is scheduled by the physician in the outpatient clinic, if required. The patient is given a urination table of 48 hours and she/he is demanded to fill it in himself/herself preferably. If necessary, enema is recommended before operation for colon cleansing. Since the urine should be sterile before urodynamics, urinalysis may need to be performed again before the appointment date, if it is scheduled for a further date.

Urethral dilatation is the procedure of expanding the last part of urinary tract undergeneral anesthesia by using particular tools. It is used in treating congenital or acquired urethral strictures and certain types of functional disorders of urinary bladder.

Considerations at Outpatient Surgery Center

Please accompany your child while she/he gets undressed and puts on special clothing.

Please be sure that there are no accessories such as watches, necklaces, rings, earrings, etc. on your child.

Please return to the waiting lounge, while the child is taken inside with stretcher.

After being kept in the reanimation unit for a while after the surgery the child will be taken to a different hall where his/her relative can be with him/her.

When the child is entirely awake, you will be allowed to leave the hospital.

It can also be reached by entering into Hacettepe İhsan Doğramacı Children’s Hospital building from door no. 1, turning left from the door across the information desk, turning the first left and moving forward on the ground floor and going down 1.5 floors from the stairs at the end of the hallway on the right. In both ways, you reach Pediatric Surgery Outpatient clinic Hall first. In this hall, there is the desk of policlinic secretariat desk and there are archive room, patient examination rooms (2) of the outpatient clinic, manometry and urodynamics room, classroom, small surgical intervention room, restrooms and research laboratory behind the desk. You can reach the secretariat of the subdivision, rooms and offices of the academic members from the short hallway opening to the hall.
